Early antibiotics don't prevent rupture, that's why immediate appendicectomy is the recommended treatment of acute appendicitis. Immediate appendicectomy is done to prevent the known complication of rupture. Appendicular rupture Occurs most frequently distal to the point of luminal obstruction along the antimesenteric border of appendix. Rupture should be suspected in the presence of fever greater than 39°C (102°F) and a white blood cell count greater than 18,000/mm3. Risk factors for perforation of appendix Extremes of age (<5 and > 65yrs) -  Fecolith obstruction Immunosuppression - Pelvic appendix Diabetes mellitus - Previous abd. surgery
