Extracellular inclusions CANNOT be found in absence of intracellular inclusions. Neuritic plaques and neurofibrillary changes are found in all associated areas of the cerebral coex. Number of neuritic senile plaques correlate with age. Neuritic plaques can be seen in the normal aged brain also, so they are non-pathogenic in all cases. Neurofibrillary tangles and neuronal alterations correlate best with the severity of dementia, not neuritic plaques. Tau proteins are formed intracellularly causing neuronal degeneration leading to Alzheimer disease.
