36 year old man, soccer injury. What needs to be performed urgently?

There are fractures of the medial and lateral malleoli. The talus is dislocated laterally, relative to the distal tibia. Manipulation to improve alignment. This ankle dislocation needs to be urgently reduced - the longer the ankle is left like this, the more severe the associated soft tissue swelling. This swelling can be so severe that if surgery were performed, it would not be possible to close the wounds.
