As the degree of external rotation (minimal or gross) has not been mentioned, here we have only one clue, i.e. the age of patient. Age of 65 years indicates more towards neck fracture. IT fracture occurs in 10-12 years elder age group than neck fracture. However, only by age you cannot differentiate between two fractures by 100% surety. Note : In later stage of fracture neck femur and IT fracture, there is flexion and adduction deformity also. In this question, the limb is an extension. It may be in early stage. However, most of the time, the limb is in flexion.
