Myositis ossificans occurs after injury to muscle fibers, connective tissue, blood vessels and underlying periosteum (not tendon rupture). Injury results in either an organizing hematoma within the muscle or implantation of injured periosteum into the adjacent skeletal muscle and the osteogenic cells escape into the muscles. These osteogenic cells cause ossification of hematoma or implanted periosteum starts ossification (cambium layer of periosteum has osteogenic potential), i.e. deposition hydroxyapatite . By this mechanism, myositis ossificans occurs in the muscle.
