Pioglitazone is a thiazolidinedione. It acts as selective agonists for the nuclear peroxisome proliferator receptor gamma and enhance the transcription of several insulin responsive genes. Activation of genes regulate fatty acid metabolism and lipogenesis in adipose tissue contribute to insulin sensitizing action. Pioglitazone is metabolized by CYP2C8 and CYP3A4. Pioglitazone when given in hypeensive patients were found to have an enhancement of left ventricular diastolic function, so its avoided in cases of diastolic dysfunction.
