Causes of Polymorphic VT ( Torsades de pointes): Hypomagnesemia Hypokalemia Hypocalcemia SAH Intracranial Hemorrhage Hereditary long QT syndrome. Note Chemical cardioversion: IV Ibutilide is done in Atrial fibrillation and atrial flutter and not in VT
