Diagnostic of bilateral pontine damage, usually from thrombosis of the basilar aery. In contrast, ocular dipping is slower arrhythmic downward movement followed by faster upward movement in patients with a normal reflex horizontal gaze. It indicates diffuse coical anoxic damage.
